Description
Blended Case Management (BCM) is a community-based program to coordinate psychiatric, medical, social, and education services for children and adolescents. Blended case managers help those with a serious mental illness or emotional disorder get the best treatment and support in the
area.
Services Provided
BCM is a short-term service that helps your child live a full, healthy life by linking them to the resources needed. We’ll help set up a recovery plan for your child based on personal goals and needs identified.
A case manager supports and assists you and your child while you’re:
- Arranging and getting mental health and medical treatment.
- Finding and securing suitable housing and/or transportation.
- Applying for benefits.
- Accessing public transit programs and services.
- Learning about other community resources to support your child’s recovery and reaching their goals.
Eligibility
We serve any child or adolescent in our service area whose mental health issues include a need for specialized coordination of services.
Examples include:
- Recent mental health hospital admissions.
- Problems keeping appointments or managing medication.
- Involvement with child welfare services or other community services.
- Using multiple community and/or mental health services.
- Needing support to meet housing or other basic needs.
- Serious problems managing life or social stressors.
Referral Process
Referrals for Blended Case Management are accepted through a variety of sources such as:
- Schools
- Community Agencies
- Families
- Managed Care Organizations
These services are funded through Pennsylvania Department of Public Welfare Medical Assistance programs. Families are required to obtain an active Medical Assistance card.
Client Rights
You have the right to:
- Considerate, respectful treatment in a safe and caring environment.
- Nondiscriminatory treatment regardless of sex, race, religion, creed, color, national origin or disability.
- Services provided in an accessible location
- Active participation in your own service and service plan.
- Full information about the service, goals, expected benefits, risks and alternatives.
- Ask questions concerning program components at any time.
- Expect confidentiality of any service information. This will be fully explained by the program staff.
Client/Family Responsibilities
- help develop service goals and participate actively in any service program.
- work toward goals in cooperation with the staff.
- be a part of the decision-making process.
- be as honest as possible in dealing with the staff.
- attend your scheduled appointments on a regular basis and to arrive on time.
- maintain confidentiality about others.
- respect the rights and property of others.
